What to Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. Take some time to review our staff and familiarize yourself with the doctors. We look forward to your visit!
Because our schedules can fill up quickly, please call early for the best selection of appointment times. If possible, arrive about 15 minutes early for your first visit to allow time for completing your records. You should bring your driver’s license or identification card and medical insurance card. If you have any copies of lab reports, x-ray studies, or other relevant medical records, please bring this information to your appointment. Please let us know about any health problems, allergies, and medications that you are taking. Your copay and any outstanding balance will be collected prior to seeing the physician.
Release of Information

Because this policy is enforced regardless of the circumstances, we may request you to sign a “Release of Information” form before we will copy any information contained within your medical chart to any third party, including but not limited to other physicians and insurance companies.
