During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. Patient Registration
Because our schedules can fill up quickly, please call early for the best selection of an appointment times. If possible, arrive about 15 minutes early for your first visit so that we can complete your records. You should bring your driver’s license or identification card and medical insurance card. If you have any copies of lab reports, x-ray studies, or other relevant medical records, please bring this information to your appointment. Please let us know about any health problems, allergies, and medications that you are taking. Your copay and any outstanding balance will be collected prior to seeing the physician.
Release of Information
All medical information pertaining to each patient seen in our office is personal and confidential. Unless you give us written permission, we will not release your records at any time. Because, this policy is enforced regardless of the circumstances, we may request you to sign a “Release of Information” form before we will copy any information contained within your medical chart to any third party, including but not limited to, other physicians and insurance companies.
