Tired of spending hours at the doctors office? Speed it up!

If you want to spend less time in the waiting room filling out forms, do it at home!  Before your first visit to Cross Timbers ENT, you will need to fill out two forms, the “registration form” and the “history form”.  You have two options:

Option #1 (you must have Microsoft Word to use this option):

Step 1: Download the two forms below.

            Form #1:  Registration form for adults or for children
            Form #2:  History form

Step 2: Fill them out using Microsoft Word.  (PLEASE don’t fill them out by hand, as many of the spaces are too small to fill out this way). 

Step 3: Save the forms and email both of them to us at:  admin@crosstimbersent.com

Step 4: Come to your appointment on time and when they ask you to fill out any
forms, say, “I did it online!”  (You will need to complete a short “Contact Authorization” form, but it only requires your name, date, and signature).

Option #2:

Step 1: Check to see if you have Adobe reader on your computer.  If you don’t have it, you can download it for free from this website:

Step 2: Download the forms below and print them out.
           
                        Form #1:  Registration form for adults or for children
                        Form #2:  History form
                       
Step 3: Fill out the forms by hand.

Step 4: You can then either fax them to us at 817-274-4292 or bring them to your appointment.

Step 5: Come to your appointment on time and when they ask you to fill out any forms, say, “I faxed them to you already!” or say, “I filled them out at home.  Here they are!”  (You will need to complete a short “Contact Authorization” form, but it only requires your name, date, and signature).
